Some basic mistakes I've seen webhosts make - feel free to add your own.
1. Billing not set up properly - so you can't even order from the site
2. Billing continues after cancellation
3. Client address not removed from e-mail alerts even when left
4. Not communicating with clients when there's a problem
5. Outsourcing support, who reply to tickets quickly, but fail to answer questions
7. Creating at least 3 different sets of lognis to use the service - Cpanel, Support, and Billing (many do this - integrate, damn you!!)
8. No social sills - replies to questions are met with single words or monotone sentence, ie Q. Can I do xxx? A: No you can't. etc
